Ans:

There are several causes for infertility. It may be male factor, female factor or both.  In males any obstructions in the vas deference or epididymis (the tubes that transport fertile sperm, absence of sperms or low sperm count, poor quality of sperms that is low motility, morphological abnormalities, liqufaction problems, antisperm antibodies etc are some of the common causes. Abnormalities in semen may be due to multiple reasons. Varicocele is one of the most common causes leading to infertility.

In females, Lack of ovulation, various ovulation problems, endometriosis, poor quality of egg, polycystic ovarian syndrome, block in the fallopian tubes, complication of other diseases etc can cause infertility. Sometimes both husband and wife may have problems.

Premature ejaculationQ:

I am 42 now and married 15yrs back,i am facing PE for long time.from the very early age i used to masturbate excessively. These 15yrs i am facing this problem, we are spending a good time for foreplay before intercourse,but when the time of intercourse within seconds ejaculation happens. I think my penis is over sensitive. can i treat this problem completely? I an in Gulf now, i will get one month leave in 1 year, is this time is enough for my treatment? I am planning to come next April.

Some people get habituated to ejaculate fast by regular practice of masturbation. But there are different causes. There is effective treatment for premature ejaculation and can be cured. Ans:

It seems that you have premature ejaculation and erectile dysfunction. Probably the spray you have used contains anaesthetic medicines. It will further worsen your erection. You need to undergo complete evaluation to find out the cause of these problems and treat accordingly. It can be treated successfully.

It appears that you are suffering from erectile dysfunction.There are multiple causes for erectile dysfunction. It can be broadly classified into organic and psychogenic causes. Organic causes include the presence of any systemic illness, side effects of medicines, hormone imbalances, problems with the blood supply to the penis etc. Psychogenic causes include performance anxiety, unhealthy mental state, depression, anxiety, traumatic experiences, relationship problems with wife etc. From the letter it appears that you have a difficulty to sustain the erection which you get initially. This indicates two possibilities. Either it is due to a psychogenic cause or a VOD. What ever may be the cause there is a definite treatment. It may be sex therapy, medicines, a combination of both or a surgery. Treatment can be decided only after identifying the exact cause. So you may consult a doctor and take an opinion. In your case it may be better to go to a hospital where all facilities are available. Poor sperm count, sperm motility & varicoceleQ:

Now my sperm count is only 1.2 million/ml, active motile sperms-0%, sluggish motile-2%, dead-98%, morphologically normal-43%. Is there any treatment to correct this? We got married in January 2006. Since then we have been trying to have a baby. But wife didn t conceive so far. We started infertility treatment in April 2007, consulted a Gynecologist at our place. That time my sperm count was 42 million/ml and motility (active) 11%. She gave medicine for both of us. After almost one year of treatment she referred us to an infertility centre. By that time my sperm count reduced from 42 to 16 million/ml and motility from 11 to 3%. Wife didn t have any problems. How ever some medicines were given. Doctor gave me HCG injections and other tablets to increase the count and motility. It increased to 23 million with 9% motility. IUI done 3 times, but she didn t conceive. Mean while I consulted a local physician. After examining physician told me that I got enlarged veins in the testis and advised to consult a surgeon for operation. But when I discussed it with the infertility specialist he advised not to go for any operations as it can produce more damage. So we decided not to do it. Doctor advised to do ICSI which costs about 2 to 2.5 lakhs. As I couldn t afford the cost we didn t go for it. Both of us got fed up, stopped the treatment for a few months. Then I took Homeo and Ayurvedic medicines for a few months.

 

Unfortunately now my count and motility reduced drastically. Now is there any chance of having a baby other than ICSI? Can I father a child? I feel that there is no point in living. Often I brood over suicide. Can any one save us? If you don t mind please give me guidance.

Ans:

It appears that you may have varicocele which was identified by the physician and referred to the surgeon for operation. You should have undergone operation. Had it been done, your sperm count and motility should have increased. In 2007 your sperm count was 42 million with 11% active motile. Now it reduced to 1.2 million with no active motile sperms and only 2% sluggish motile. It shows a significant reduction in the sperm count and motility over the last 4 years. Varicocele causes progressive and duration dependent damage to the testis. As a result sperm motility will be reduced first followed by count. Over the years testis become atrophied and the production of male hormone (testosterone) will reduce. It will cause loss of sexual desire and erection. In other words you may develop an early andropause.

If there is a varicocele in an infertile man or in an unmarried man or a person who doesn’t have children, one must do microsurgical varicocelectomy. This is the latest scientific view and standard teaching on this matter. How ever in most of the places varicocele is usually operated using laparoscopic surgery or conventional surgery.

The most common complications of conventional varicocele operations are hydrocele, varicocele recurrence, and injury to testicular artery, spermatic cords etc. This may the reason why your Gynecologist advised against the surgery and advised for ICSI. A person having varicocele should undergo microsurgical varicocelectomy before going for IUI, IVF or ICSI to improve the sperm quality. It will reduce or nullify the adverse effect of ROS (reactive oxygen species) on the sperms. Presence of varicocele will reduce the percentage of success when going for artificial reproduction techniques.

In microsurgery the use of the operating microscope allows for reliable identification of spermatic cord, lymphatics, internal spermatic veins and venous collaterals, and the testicular artery or other arteries. So that chances for these complications are almost reduced to less than 1 percent. Repair of varicocele results in greater improvement in sperm motility and count.

Microsurgical varicocelectomy provides a safe and effective approach to varicocele repair with preservation of testicular function, improvements in semen quality, and improvements in pregnancy rates.

Hence don’t get disappointed, there is definite chance for recovery and to have baby by normal sexual intercourse without going for ICSI or IVF.

Hypertension & premature ejaculationQ:

Iam 45 years old male with two children. I have high Bp andis on a regular medications for the same. After taking this medicine ( Amlong 5mg) i am having premature ejaculation. Is i due to this medicine or due to age factor.

Ans:

Amlodipine doesn t cause premature ejaculation. Most probably it could be due to some other reasons such as an inflammation of prostate gland or seminal vesicles. Any infection of these gland or urinary bladder, anxiety etc. It can be treated and cured. You can consult an andrologist who has sufficient experience to treat such problems.
