Varicocele operation, best methodQ:

Dear Doctor, My name is suresh and i am basically from Trivandrum.Now i am in Australia and i have some doubts regarding the Microsurgical Varicocelectomy I have been suffering from testicular pain for last one year and consulted a urologist 3 months back and according to scan report , semen analysis,,FSH and LH i have diagnosed bilateral varicocele. So the Doctor advised for "varicocelectomy" and i heard that "microsurgical varicocelectomy "is the latest technology using for this. I would like to do this at your facility and could you please advise me more about this ie, do i have to take prior appointment and do all the tests again etc.. thanks suresh

Dear Mr Suresh,
Varicocele is one of the common causes of testicular pain. The operation for the varicocele is usually done using laproscopic surgery, conventional TV ligation or microscopic microsurgical varicolectomy. Laproscopic surgery is the least time consuming and easy procedure and the patient can leave the hospital next day. It is easy for the doctor and more convenient for the patient as the surgery can be performed within half an hour and the patient doesn’t need much rest. The major disadvantage of the laparoscoic operation is that on each side one dilated vein will be ligated, the remaining ones are left out. In the conventional TV ligation only the enlarged veins visible to the naked eye, are ligated.  In Varicocele there may be several dialated veins (average 5 to 12) on each side such as the internal and external spermatic veins, cremasteric veins etc.  All these veins need to be ligated and other structures like arteries and lymphatics have to be preserved without causing any damage. The most common complications of varicocelectomy are varicocele recurrence, hydrocele and injury to arteries. When using an operating microscope or loupe these structures are clearly visible and complications can be reduced significantly. Hence at Dr. Promodu’s Institute, we perform only microsurcgical microscopic varicocelectomy. When you come for consultation please bring all your investigation reports. Ans:

Homosexuality is of two types. The first is ego syntonic in which the individual enjoy and it doesn't cause any mental stress to the person. Second type is ego dystonic in which the individual feels mental agony and distress about his own homosexual behaviour and hence wants to change it. No treatment is indicated for the first group. Ego dystonic homosexuality can be treated if the individual has a genuine motivation to change his behaviour.

Treatment is indicated only if the person has any discomfort or problem due to his own homosexual tendency or behaviour. The desire to change the orientation should not be due to other's (family, friends, religion etc) pressure. Treatment will be successful only if the person has a very genuine and consistent motivation to change his behavior. There is no medicine to change the sexual orientation. Treatment is psychological using techniques of sex therapy based on cognitive behavioural principles. Usually we don't take any homosexuals for treatment. But we have treated a few men with genuine motivation and they recovered.

Ans:

Usually night emission is not considered as a disease. When a boy reaches the adolescent age night emission begins as a sign of maturity.  It indicates that he has achieved reproductive ability. The frequency is more till the age of 25 to 30 yrs, after that frequency decreases. If the person engages in regular masturbation or sexual contacts night emission may not occur very frequently. If there is any pain, burning sensation or discomfort after ejaculation there may be a chance for infection or inflammation of prostate gland or seminal vesicles. In such condition treatment is required.

Dear sir, I am a man of 43 years, my problem is that I have short penis around 5 and half inches when erected. Can we increase the size of penis up to 7 inches without surgery by using medicines? It appears from your file that you have not undergone the scanning(CDDU) as advised by the doctor. Unless all the investigations are completed as per doctors advice no further treatment is possible.

Often it is psychological problem. Usually people with depression are not able to achieve an orgasm. Problems like depressive cognition, negative attitude, mental stress, obsessive traits etc can impair the orgasmic response. Normally, it is a difficult condition to treat.
