From our experience of treating thousands of patients with sexual problems from Kerala and outside it is found that 99% of men complaints of small penis got apprehension and doubt about the size of their sexual organ. Ans:

It appears that you are suffering from premature ejaculation. This may be due to various causes such as infection, inflammation, hypersensitivity, habituation or any other factors. First we have to find out the cause. Treatment may vary from medication to sex therapy or a combination of both based on the causes. FrenuloplastyQ:

my forskin is very tight end of penis neck so i cannot to move back the skin, what should i do? is surgery facility avilable in your clinic?

Ans:

From your description it appears that you may have a tight frenulum.  It can cause difficulty for sexual intercourse. Vaginal penetration may become difficult and you are likely to experience pain. Sometimes there can be small injury and a little bleeding if you try to insert the penis by force.  If it is a tight frenulum it can be corrected by a simple surgery called Frenuloplasty. In certain cases circumcission may be required. Without examination an opinion cannot be given.

We do this surgery routinely in this hospital.

There are multiple causes for erectile dysfunction. It can be broadly classified into organic and psychogenic causes. Organic causes include the presence of any systemic illness, side effects of medicines, hormone imbalances, problems with the blood supply to the penis etc. Psychogenic causes include performance anxiety, unhealthy mental state, depression, anxiety, traumatic experiences, relationship problems with wife etc. From the letter it appears that you have a difficulty to sustain the erection which you get initially. This indicates two possibilities. Either it is due to a psychogenic cause or a VOD. What ever may be the cause there is a definite treatment. It may be sex therapy, medicines, a combination of both or a surgery. Treatment can be decided only after identifying the exact cause. So you may consult a doctor and take an opinion. In your case it may be better to go to a hospital where all facilities are available.

Usually night emission is not considered as a disease. When a boy reaches the adolescent age night emission begins as a sign of maturity.  It indicates that he has achieved reproductive ability. The frequency is more till the age of 25 to 30 yrs, after that frequency decreases. If the person engages in regular masturbation or sexual contacts night emission may not occur very frequently. If there is any pain, burning sensation or discomfort after ejaculation there may be a chance for infection or inflammation of prostate gland or seminal vesicles. In such condition treatment is required.
