We have been married for 7 years and for the first 5 years there has been no problems. In fact for the last 1.5 years we had sex only 2-3 times. My wife has been willfully refusing me sex, citing reasons that she is tired. We have 2 young sons, and on most of the days she deliberately delays sleeping of the children so as to avoid having sex with me. I asked her regarding this and she just said she does not have the mood for it. There is nothing physically wrong with her. I feel the problem is psychological. I suggested that we consult a doctor, but she refused saying that the problem is mine and not hers. I feel she is hiding something from me. She is always on the mobile phone talking to other men, but does not disclose properly to whom she is talking to. I suspect whether she is having an extra marital affair. I think she refuses to see doctor because she fears that the truth will come out if the doctor questions her. Please tell me what should I do. Do you people provide marriage counseling ? If yes, then whom should I contact for this. I think it would be impossible to bring my wife there for counselling, so the only way would be for someone to visit our house at Ernakulam and forcefully ask her what is her problem. Anyway please suggest what should I do.
We neither provide any online consultation nor in situ.

If a married man living with wife does masturbation regularly instead of having sexual contact with wife it may lead to marital discord and even divorce. For unmarried people or those who lives alone it is considered as a normal activity that will help to release sexual tension. In certain people fast regular masturbation may cause premature ejaculation by habituation. Usually masturbation doesn't cause any other health problems.

Erectile dysfunction is treatable. Treatment varies from sex therapy, medicines or surgery. It can be planned only after appropriate investigations and physical examination. Without this we are unable to give an opinion.

I have pain in my testils and low semen count what i do?
It appears that you are suffering from Varicocele
Varicocele causes progressive and duration dependent damage to the testis. As a result sperm motility will be reduced first followed by count. Over the years testis become atrophied and the production of male hormone (testosterone) will reduce. It will cause loss of sexual desire and erection. In other words you may develop an early andropause.
If there is a varicocele in an unmarried man one must undergo microsurgical varicocelectomy. This is the latest scientific view and standard teaching on this matter. How ever in most of the places varicocele is usually operated using laparoscopic surgery or conventional surgery.
The most common complications of conventional varicocele operations are hydrocele, varicocele recurrence, and injury to testicular artery, spermatic cords etc. This may the reason why your Gynecologist advised against the surgery and advised for ICSI. A person having varicocele should undergo microsurgical varicocelectomy before going for IUI, IVF or ICSI to improve the sperm quality. It will reduce or nullify the adverse effect of ROS (reactive oxygen species) on the sperms. Presence of varicocele will reduce the percentage of success when going for artificial reproduction techniques.
In microsurgery the use of the operating microscope allows for reliable identification of spermatic cord, lymphatics, internal spermatic veins and venous collaterals, and the testicular artery or other arteries. So that chances for these complications are almost reduced to less than 1 percent. Repair of varicocele results in greater improvement in sperm motility and count.
Microsurgical varicocelectomy provides a safe and effective approach to varicocele repair with preservation of testicular function, improvements in semen quality, and improvements in pregnancy rates. So in your case the best solution is bilateral microsurgical varicocelectomy.

We have seen and successfully treated so many couples married for several years without having sexual intercourse. This condition is called unconsummated marriage. The couple with longest duration I have seen is of 22 years. Non consummation of marriage can occur due to several reasons. It is due to problems related to either husband, wife or both. Male problems such as erectile dysfunction, premature ejaculation, Sexual desire disorder, dyspareunia, phimosis, peyronie s disease and female problems such as absence of sexual desire, sexual arousal disorder, dyspareunia, sexual aversion disorder, vaginismus etc can cause non consummation of marriage. Other psychological problems such as adjustment problems, marital conflicts or dislike towards the partner also can lead to such conditions. How ever nothing to worry about it. With modern investigative and treatment modalities we can identify the cause and treat it successfully.

Hello Doctor, I am married since 2011 August and it is going to be 1 year this August. We both are happy with each other and we feel as though we are made for each other. The only problem between us is we have not had sex until now. Our love and affection leads up to cuddling but could not take till a sex. During the initial few months of wedding we had tried for the same but as my husband s foreskin is not freely moving he finds it very painful. Gradually he is loosing confidence and he is not able to keep himself on for long. We understand that this is a very minor issue. But the problem is he is a kind of a person who is even scared of injections. So he is not getting mentally convinced to go and meet a doctor. He is not co-operating to find a solution for this issue. Please advice what could be done in this situation. Thanks Gayathri...
