I am 40 years old and I have mild varicoceles. I have a child of 10 years and want to have another. But the semen parameters are not okay. Please advise the best option.
Varicocele is the commonest cause of male infertility. If you are planning for a second child it is important to correct your varicocele. After the correction if all other parameteres of your wife and you are normal, your wife will conceive in the normal way. There may not be any need for the ART methods such as ICSI or IVF. Comparitively side effects are less for this medicine. Even if it is due to a particular medicines one must recover after stopping the same.

The girth of my penis at the erect position is is just 3.5 inches. My wife is not comfortable with me. Can girth be increased with medicine, water pump or through any other means?
There is no medicines to increase the girth of penis. Only surgical methods are available.
Your wifes lack of satisfacion can be due to various other reasons also. If there is any such problems that has to be corrected before planning for any surgery to enhance the girth of your penis. I think both of you needs a detailed evaluation before planning any treatment.

Venous leak is treatable. We cannot consider it just like an infection or a diarrhea which is curable.
1. If you suspect venous leak we have to test it and confirm.
2. Then treat with medicines and see whether you are able to maintain an erection sufficient for a satisfactory sexual intercourse.
3. If medicines are not effective the choice of treatment is surgery either venous ligation or implant.
